At the recent Working Together to Transform End of Life Care in Acute Hospitals national event, we shared with you our newly revised, Transforming end of life care in acute hospitals: The route to success ‘how to’ guide.  I am delighted to confirm that this valuable resource is now available online for you to share with your colleagues.

The publication of this version of the Guide, following the withdrawal of the Liverpool Care Pathway, marks a significant milestone in improving end of life care for patients and their families.  The guide’s revision has been led by the End of Life Care programme team at the Sustainable Improvement Team, NHS England, with contributions from The National Council for Palliative Care,  Macmillan Cancer Support and NHS Trust Development Authority. as an important component of our new partnership for improving end of life care in acute hospitals.

Building on the legacy work from NHS Improving Quality, the Guide provides practical advice and support for frontline clinicians and leaders on ‘how to’ identify, lead and implement change for themselves to improve their local services in collaboration with their local partner organisations across their community.
